Report It!
FSCJ faculty, staff, and students play a role in promoting the safety and well-being of our community. The page provides guidance to assist college community in selecting the appropriate reports/requests. Use of this form ensures that various concerns are routed to appropriate college personnel. Below is a summary of the reporting options included on Report It! Please click the link under the report or request you would like to submit.
Academic Early Alert Report
Complete this form to report academic concerns or college transition concerns which may impact a student’s academic experience. This could include frequent absences, lack of preparation, or poor performance in the classroom. Submit an Early Alert Report.
CARE Report for Students of Concern
Complete the Care Report form to report behavioral concerns about students. A behavioral concern may be reported when a student exhibits behaviors such as depression, anxiety, disorientation, alcohol and drug addiction or thoughts of harming self, and could potentially be a risk to self and others. If you submit a CARE Report, you may be contacted for additional information or follow up. For concerns that require immediate medical or emergency response, contact campus security or 9-1-1. Submit a CARE Report.
Title IX Report
Complete this form to report an alleged act of gender discrimination, sexual harassment, or sexual misconduct (including sexual assault, rape, stalking, relationship violence, dating and domestic violence, and sexual exploitation). Submit a Title IX Report.
Student Conduct Report
Complete this form when a student has demonstrated an act or behavior that impairs, interferes with, or otherwise disturbs or obstructs the orderly functions, and/or interests of the entire FSCJ community. Submit a Student Conduct Report.
Academic Honesty Report
Complete this form to report when there has been an incident of cheating or plagiarism on class work including assignments and tests. Submit an Academic Honesty Report.
Brainfuse Faculty Dashboard
