Health Insurance and Insights from Health Literacy by National Academies of Sciences, Engineering, and Medicine; Health and Medicine Division; Board on Population Health and Public Health Practice; Roundtable on Health Literacy; Joe Alper (Editor)Since the passage of the Patient Protection and Affordable Care Act (ACA), health care reform has created major changes in the U.S. health care system. The ACA has brought millions of people into the system who had no previous access, and many of these newly enrolled individuals have had limited experience navigating the complex and complicated U.S. health system. In July 2016 the National Academies of Sciences, Engineering, and Medicine convened a public workshop to examine health insurance through the lens of health literacy, focusing on literacy related barriers to information and coverage as well as on possible solutions. Participants discussed the role of health literacy in accessing health care and remaining in treatment; delivery and financing system reforms that affect organizational health literacy; and quality and equity considerations. The Core Elements of Value in Healthcare by Paveljit BindraInstructor Resources: Test bank, PowerPoint slides, and case studies with discussion questions and teaching notes. When you go shopping, do you simply go to the store with the intention of buying as many items as you can? Or do you select items based on the quality and satisfaction they will deliver for the money you spend? For most people, the quality of a product, relative to its cost, is most important. The field of healthcare today is making a similar distinction, placing a heightened emphasis on the delivery of value as opposed to sheer volume. So what exactly does it mean to deliver value in healthcare? For decades, providers in the United States operated according to a model of volume-based care, under the general assumption that more care was better. In recent years, however, important developments in policy and payment and innovations in disease management, complex case management, population health, predictive analytics, and decision support have contributed to a paradigm shift toward value-based care. Organizations are now striving to eliminate waste, deliver higher-quality care at a lower cost, and increase patient satisfaction. The Core Elements of Value in Healthcare provides a comprehensive introduction to value in healthcare as it relates to policy, delivery, finance, economics, strategy, public health, population health, operations, compliance, managed care, accountable care, and preventive medicine. The concepts of this book are reinforced through case examples and detailed discussion questions that encourage readers to apply the key principles as they learn. Drawing from his experience as a physician, a hospital executive, and the founder of a medical group and health plan, Dr. Paveljit S. Bindra examines value from a variety of perspectives. Specific areas of focus include the following: Clinical innovationsAlignment structures Payment reform Quality improvement Information technology The Core Elements of Value in Healthcare equips readers with the knowledge to address disparate elements in the healthcare value chain, and it demonstrates how effective coordination of these elements can enhance the quality of care, reduce costs, and improve patient satisfaction and outcomes.

Private Health Insurance by Peter HawkinsMost Americans are covered by private health insurance, which they either obtain through employment or purchase individually. Insurance premiums -- the payments made to buy that coverage by enrollees or by other parties on their behalf -- are high and rising. The Congressional Budget Office and the staff of the Joint Committee on Taxation (JCT) projected that in 2016, the average premium for an employment-based insurance plan will be about $6,400 for single coverage and about $15,500 for family coverage. Average premiums for coverage purchased individually (in what is called the non-group market) are also high -- but not quite as high as average employment-based premiums, mostly because non-group coverage is less extensive and thus requires enrollees to make higher out-of-pocket payments when they receive care. This book reviews the factors affecting premiums, federal policies and tax credits in private health insurances.

The Ethics of Universal Health Insurance by Alex RajcziMillions of Americans lack health insurance, and as a result, thousands suffer and die every year. Philosophers have argued that an ideal society would avoid these problems by guaranteeing reasonable universal access to health insurance-but what about concerns that a universal health insurancesystem would be inefficient, create excessive fiscal risk, or impose high taxes and other personal costs? In The Ethics of Universal Health Insurance, Alex Rajczi shows how defenders of universal health insurance can address the ethical issues raised by these objections and make the moral case for an American universal health insurance system that improves on the gains made in the Affordable Care Act.By addressing these objections, Rajczi reveals fascinating philosophical issues that have been previously overlooked. Understanding these issues in the health care debate has larger upshots as well. This book will help readers form a deeper and more thorough understanding of progressive andconservative views on distributive justice, and it will provide a framework for examining debates about any part of the social safety net-in America and elsewhere.

Exposed by Christopher T. RobertsonA sharp expos?? of the roots of the cost-exposure consensus in American health care that shows how the next wave of reform can secure real access and efficiency. The toxic battle over how to reshape American health care has overshadowed the underlying bipartisan agreement that health insurance coverage should be incomplete. Both Democrats and Republicans expect patients to bear a substantial portion of health care costs through deductibles, copayments, and coinsurance. In theory this strategy empowers patients to make cost-benefit tradeoffs, encourages thrift and efficiency in a system rife with waste, and defends against the moral hazard that can arise from insurance. But in fact, as Christopher T. Robertson reveals, this cost-exposure consensus keeps people from valuable care, causes widespread anxiety, and drives many patients and their families into bankruptcy and foreclosure. Marshalling a decade of research, Exposed offers an alternative framework that takes us back to the core purpose of insurance: pooling resources to provide individuals access to care that would otherwise be unaffordable. Robertson shows how the cost-exposure consensus has changed the meaning and experience of health care and exchanged one form of moral hazard for another. He also provides avenues of reform. If cost exposure remains a primary strategy, physicians, hospitals, and other providers must be held legally responsible for communicating those costs to patients, and insurance companies should scale cost exposure to individuals' ability to pay. New and more promising models are on the horizon, if only we would let go our misguided embrace of incomplete insurance.

Community-Based Health Literacy Interventions by National Academies of Sciences, Engineering, and Medicine; Health and Medicine Division; Board on Population Health and Public Health Practice; Roundtable on Health Literacy; Joe Alper (Editor)In its landmark report, Health Literacy: A Prescription to End Confusion, the Institute of Medicine noted that there are 90 million adults in the United States with limited health literacy who cannot fully benefit from what the health and health care systems have to offer. Since the release of that report, health literacy has become a vibrant research field that has developed and disseminated a wide range of tools and practices that have helped organizations, ranging in size from large health care systems to individual health care providers and pharmacists, to engage in health literate discussions with and provide health literate materials for patients and family members. Improving the health literacy of organizations can be an important component of addressing the social determinants of health and achieving the triple aim of improving the patient experience, improving the health of populations, and reducing the cost of care. However, the focus on organizations does not address the larger issue of how to improve health literacy across the U.S. population. To get a better understanding of the state of community-based health literacy interventions, the Roundtable on Health Literacy hosted a workshop on July 19, 2017 on community-based health literacy interventions. It featured examples of community-based health literacy programs, discussions on how to evaluate such programs, and the actions the field can take to embrace this larger view of health literacy. This publication summarizes the presentations and discussions from the workshop.
Health Literacy by R.A. Logan (Editor); E. R. Siegel (Editor)While health literacy is a relatively new multidisciplinary field, it is vital to the successful engagement with and communication of health with patients, caregivers, and the public. This book 'New Directions in Health Literacy Research, Theory, and Practice' provides an introduction to health literacy research and practice and highlights similar scholarship in related disciplines. The book is organized as follows: the first chapter explains the still-evolving definition of health literacy; the next three chapters discuss developments and new directions in health literacy research, then a further two chapters are devoted to developments and new directions in health literacy theory. Two chapters explore health literacy interventions for vulnerable populations; four chapters cover health literacy leadership efforts; six chapters describe developments and new directions in disciplines that are similar to health literacy; and six chapters portray diverse health literacy practices. A preface from Richard Carmona M.D., the former U.S. Surgeon General, is included in the book.Although the book is intended primarily for health literacy researchers, practitioners and students, the diverse topics and approaches covered will be of interest to all healthcare and public health researchers, practitioners, and students, as well as scholars in related fields, such as health communication, science communication, consumer health informatics, library science, health disparities, and mass communication. As Dr. Carmona concludes in his preface: 'This is essential reading for all health practitioners.'

The Role of Emotions in Preventative Health Communication by Jessica Gall MyrickHealth-related media permeate our modern experience, from using an online search engine to reading a pamphlet about vaccinations at the doctor's office or watching a television news report on the dangers of sitting too much. This book makes the argument that if prevention-focused health messages are to motivate behavior change, they must tug at the heartstrings, and researchers need to understand more precisely how different emotional reactions influence health message effects. In making this case, this book takes a quantitative, social science-based approach to understanding the role of emotions in shaping individual-level effects to preventative health messages disseminated through mass media channels. The book focuses on how discrete emotions evoked by preventative health media messages influence how audiences respond to those messages. Are they persuaded to change their behavior? Will they seek more information? Will they share information with others? Will they support prevention-focused policies? While a rich literature exists on the effects of health-related fear appeals on audiences, researchers have yet to fully explore the role that other discrete emotions play in health communication processes and outcomes. This book fills that gap by providing an overview of the role of nine different emotions--both positive and negative--in various prevention-focused health communication settings. It also introduces readers to commonly employed emotional theories and concepts and relates them to literature on prevention-focused health and policy communication. In addition to reviewing and synthesizing the literature, this book offers new directions to researchers hoping to improve the effectiveness of prevention-focused health messages.

Effective Health Communication for Sustainable Development by Patchanee MalikhaoThe health communication strategies presented in this book are based on the understanding that there are many factors underpinning health. Starting from a framework of addressing health at an individual, group, and structural level, we need to realize the impact of globalization and social determinants on health. Health literacy and media literacy are necessary for health communicators and stakeholders to communicate effectively about/on health. Knowledge and skills in media technology, especially new media, new developments in the Internet and mobile devices are important. In addition, intercultural communication is essential to understand and collaborate with others. Health communication needs collaboration from all sectors in society. The health communication promoted in this book is based on a participatory model based within the multiplicity paradigm. Strategies suggested in this book are practical examples to encourage the reader to design his/her own strategies to serve a particular community. Before one can devise a strategy for health promotion and disease prevention, one needs to understand that a top-down communication approach cannot change nor sustain peoples health behaviors. On the contrary, one needs to create an enabling environment or a context that supports an individuals decision to change their behavior. How to achieve sustainability in health or health for all is ultimately what this book is about. In nine chapters, the reader is introduced to the history and discourses of health communication and sustainable development; the first part provides the reader with an in-depth overview of the many health communication theories, set in the context of globalization and localization and assessed from both anthropological and sociological perspectives. In the second part of the book, chapters systematically explain communication for/about health for sustainable development in general, as well as regarding old and new traditional and digital media in particular. The concluding chapters present the principles and applications of health communication strategies for sustainability. Sustainability in health can only be achieved once health equality is a reality.

Patient Safety by Alexander L. Gungov; Tatiana Tzarvulanova (Translator)In our time of well-publicized health care travails, in the U.S. and the UK and elsewhere, matters of financing too often subsume the dimension of patient care. In his latest book, Alexander L. Gungov studies a vital but neglected aspect of patient safety. Of the thousands of medical errors committed on a daily basis, in the bulk of unfortunate clinical decisions, a significant share pertains to various logical flows and epistemological fallacies. By focusing on the logical dimensions of clinical medicine, Gungov promotes awareness of the logical and epistemological traps that lie in the day-to-day care of patients. Such a focus not only allows us to avoid falling into them, but demonstrates the practical value of looking at medicine from a new philosophical perspective. That perspective involves a broad and unusual collection of philosophers. The discussion takes its starting point from J. S. Mill's inductive methods and Giambattista Vico's verum-factum principle, but then sets out a unique combination of Charles Sanders Peirce's abductive reasoning, Immanuel Kant's reflective judgment, as well as G. W. F. Hegel's and D. P. Verene's speculative thinking, all marshalled to present a novel philosophical account of clinical diagnostics. Interpretation of practical examples elucidate the logical aspect of medical errors and suggests strategies of overcoming them. The book as a whole demonstrates the value of Hans-Georg Gadamer's hermeneutical insights into the enigmatic character of health. This much-needed book will be of interest to medical practitioners, health policy makers, patients and their families, and advanced students and scholars in medicine, the medical humanities, medical epistemology, and the philosophy of medicine in general.

Evidence-Based Practice for Public Health Emergency Preparedness and Response by National Academies of Sciences, Engineering, and Medicine; Health and Medicine Division; Board on Population Health and Public Health Practice; Board on Health Sciences Policy; Committee on Evidence-Based Practices for Public Health Emergency Preparedness and Response; Autumn Downey (Editor); Lisa Brown (Editor); Ned Calonge (Editor)When communities face complex public health emergencies, state local, tribal, and territorial public health agencies must make difficult decisions regarding how to effectively respond. The public health emergency preparedness and response (PHEPR) system, with its multifaceted mission to prevent, protect against, quickly respond to, and recover from public health emergencies, is inherently complex and encompasses policies, organizations, and programs. Since the events of September 11, 2001, the United States has invested billions of dollars and immeasurable amounts of human capital to develop and enhance public health emergency preparedness and infrastructure to respond to a wide range of public health threats, including infectious diseases, natural disasters, and chemical, biological, radiological, and nuclear events. Despite the investments in research and the growing body of empirical literature on a range of preparedness and response capabilities and functions, there has been no national-level, comprehensive review and grading of evidence for public health emergency preparedness and response practices comparable to those utilized in medicine and other public health fields. Evidence-Based Practice for Public Health Emergency Preparedness and Response reviews the state of the evidence on PHEPR practices and the improvements necessary to move the field forward and to strengthen the PHEPR system. This publication evaluates PHEPR evidence to understand the balance of benefits and harms of PHEPR practices, with a focus on four main areas of PHEPR: engagement with and training of community-based partners to improve the outcomes of at-risk populations after public health emergencies; activation of a public health emergency operations center; communication of public health alerts and guidance to technical audiences during a public health emergency; and implementation of quarantine to reduce the spread of contagious illness.

Healthcare Teamwork by Theresa J. K. Drinka; Phillip G. ClarkBoth comprehensive and accessible, this is an ideal resource for anyone who plans to teach or practice integrated, cost-effective healthcare in the 21st century. Currently, there is no coordinated system for training health-profession students to address the needs of patients with complex illnesses, nor is there a coordinated system for effectively delivering care to these patients. This book explores both sides of the problem, bringing interprofessional practice and education together to show how they are complementary--and how they can be integrated to provide better care. In many respects, this book is a personal account of the authors' experience with interprofessional teamwork and education over the past 40 years. It discusses what works and what doesn't and includes interviews, examples, and case studies that illustrate the perspectives of healthcare professionals, patients, and caregivers. This second edition illuminates ways in which today's business model has changed interprofessional healthcare team practice and education, and it examines the needs of patients relative to healthcare teams and practitioner education. An entire chapter is devoted to the patient's position as both teacher and learner in relation to the team. The theoretical foundations of practice and education are highlighted, but the book also shares models that can be used for the practical development of programs. Explores the complexities of interprofessional teamwork and education, addressing both practice and teaching Discusses how patients are affected by healthcare providers who do not function as a cohesive team and looks at the patient's role in teamwork Offers a detailed model of interprofessional teamwork based on the authors' experience with a long-term, well-functioning interprofessional healthcare team Uses illustrative narratives and case studies to provide examples of the concepts and principles presented Includes a chapter based on interviews with patients and their caregivers to highlight experiences with functional and dysfunctional teams Presents new topics, such as critical areas of practice (primary care, long-term care, and transitions of care); ethical issues in teamwork; educational theory; the use of narrative; and challenges in sustaining interprofessional education

Culturally Competent Compassion by Irena PapadopoulosBringing together the crucially important topics of cultural competence and compassion for the first time, this book explores how to practise ¿culturally competent compassion¿ in healthcare settings ¿ that is, understanding the suffering of others and wanting to do something about it using culturally appropriate and acceptable caring interventions. This text first discusses the philosophical and religious roots of compassion before investigating notions of health, illness, culture and multicultural societies. Drawing this information together, it then introduces two invaluable frameworks for practice, one of cultural competence and one of culturally competent compassion, and applies them to care scenarios. Papadopoulos goes on to discuss: how nurses in different countries understand and provide compassion in practice; how students learn about compassion; how leaders can create and champion compassionate working environments; and how we can, and whether we should, measure compassion. Culturally Competent Compassion is essential reading for healthcare students and its combination of theoretical content and practice application provides a relevant and interesting learning experience. The innovative model for practice presented here will also be of interest to researchers exploring cultural competence and compassion in healthcare.

DSM-5 and Family Systems by Jessica A. Russo; J. Kelly Coker; Jason H. King"Two of my mantras in training systemic clinicians are: 'Systemic counselors and Marriage and Family Therapists should be the best diagnosticians of all the mental health disciplines but we do not always have to take that diagnosis into the therapy room to our clients.' and... 'Use the language of your audience.' The cognitive and vocal dissonance between the systemic understanding of human relational behaviors and the traditional medical model of viewing these same symptoms of pathology has long been a topic of academic debate and clinical coffeehouse arguments. This work is the first major text to attempt to create a two way bridge of introducing traditional users of the DSMs to systemic perspectives while informing systemic thinkers of the utilization of the traditional language for better outcomes for all of our clients/patients. Though no work can be exhaustive, this text uses the language of the DSM-5, explores systemic influences, and provides a case examples of the major divisions of the manual. I recommend this text in hopes that the bridge will continue to be used as our body of knowledge continues to grow." --Mike Bishop, PhD., LMFT-S, LPC-S; Associate Professor, Texas Wesleyan University; Former President, Texas Association for Marriage and Family Therapy "This outstanding textbook addresses the needs of systemic oriented clinicians who work with couples and families to identify and assess disorders described in the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders. It provides a much needed resource emphasizing the development of interventions from systemic frameworks. The book is characterized by chapters organized in a consistent manner that has great appeal to practitioners in a variety of settings: diagnosis and criteria, relational and cultural features, family systems assessments, family systems interventions, ethical/legal considerations, and case conceptualizations. I find the entire text to be timely and relevant but one of my favorite aspects of the book is the way each chapter concludes with a case study that applies the information to the work of a systemically oriented." clinician."--David Capuzzi, Ph.D, LPC, NCC; A Past President; American Counseling Association The first text to present DSM-5 diagnoses within a relational perspective, DSM-5 and Family Systems delivers timely content aimed at training marriage and family therapists, clinical mental health counselors, and other systems-oriented practitioners. It reflects how the DSM-5 examines, for the first time, its diagnostic categories from the perspective of cultural and environmental impact on the development of individual disorders and conditions. This comprehensive text provides students with an understanding of how to approach a diagnosis as it relates to assessments, treatment planning, and ethical implications from a family and relational systems perspective. With contributions from distinguished faculty at counseling and marriage and family therapy training programs, each chapter includes an overview of the DSM in family systems contexts, cultural aspects, family systems assessments and interventions, and ethical and legal implications. Public Health Law by Lawrence O. Gostin; Lindsay F. Wiley; Thomas R. Frieden (Foreword by)Lawrence O. Gostin's seminal Public Health Law is widely acclaimed as the definitive statement on public health law at the turn of the twenty-first century. In this bold third edition, Gostin is joined by Lindsay F. Wiley to analyze major health threats of our time such as chronic diseases, emerging infectious diseases, antimicrobial resistance, bioterrorism, natural disasters, opiod overdose, and gun violence. The authors draw on constitutional law, administrative law, local government law, and tort law to develop their conception of law as a tool for protecting the public's health. The book creates an intellectual framework for modern public health law and supports that framework with illustrations of the scientific, political, and ethical issues involved. In proposing innovative solutions for the future of the public's health, Gostin and Wiley's essential study provides a blueprint for public and political debates to come. New issues covered in this edition: * Corporate personhood rights raised in response to regulations of tobacco, food and beverages, alcohol, firearms, prescription drugs, and marijuana. * Local government authority to protect the public's health. * Deregulation and harm reduction as modes of public health law intervention. * Taxation, spending, and alteration of the socioeconomic environment as modes of public health law intervention. * Access to health care as a strategy for protecting the public's health. * Taxation, spending, licensing, zoning, and shared-use strategies for chronic disease prevention. * The public health law perspective on violence and injury prevention. * Health justice as a framework for reducing health disparities and protecting the public's health.
